POSITION DETAILS:

Schedule: Days, 8-hour shifts in person (not remote) Mon-Fri

FTE: Full-Time, 80 hours per 2 week pay period

Casual status: 6 months (covering a leave of absence)

JOB SUMMARY:

The Infectious Disease division cares for patients with general and complex diagnoses and symptoms with acute and long term diseases. The Infectious Disease Care Coordinator, in collaboration with the Infectious Disease specialty medical providers and medical assistant, performs case management and direct patient care to assigned patients. This includes point of care procedures, management of central lines, procedures in clinic, ongoing patient, family and staff education and coordination of care with outside community agencies. The coordinator also triages and manages patient requests and needs via telephone triage and patient messaging via EPIC. The coordinator also partners and supports continued growth of medical assistants to deliver well rounded care. The Coordinator facilitates effective functionality of clinic operations, actively participates in Quality Management and acts as a provider extender within his/her scope of practice. The Care Coordinator performs additional team functions as required to maintain effective customer service. The coordinator role in Infectious Disease, has a blend of hand on procedures, complex patient care coordination, education on disease management, triage, and in clinic support. We are looking for someone that is autonomous, collaborative, supportive of teams, educator, customer focused, quick learner, and solution driven. The coordinator would be joining a dynamic division with a collaborative partnership with Providers and ancillary support staff as well as an engaged and supportive team.

MINIMUM QUALIFICATIONS:

• Associate's Degree

• Two years of nursing experience

• California RN License

• BLS for Healthcare Providers, issued by AHA

PREFERRED QUALIFICATIONS:

• Area of Study in a Field Related to the Position (ID)

• Five years of experience

• Bachelor's Degree in Nursing

• Ped Adv Life Support Prov

• Bilingual – Spanish

• Ambulatory care experience

• Pediatrics experience

The current salary range for this position is $58.87 to $101.32

A candidate’s placement in the range will be determined based on the terms and conditions of the applicable collective bargaining agreement.
