    Job Description:

    + Conducts comprehensive assessments of patient and donor psychosocial, cultural, and spiritual needs through interviews, observations, and clinical data.

    + Evaluates family relationships, support systems, mental health history, personality traits, and coping skills to inform care plans.

    + Collaborates with the transplant team to develop and implement individualized care plans that address the holistic needs of patients and donors.

    + Provides counseling and support to patients and donors, facilitating decision-making and addressing concerns related to the transplant process.

    + Participates in multidisciplinary rounds and transplant team meetings to discuss patient and donor progress and care plans.

    QUALIFICATION REQUIREMENTS:

    Master's (Required)Clinical Social Worker License (LCSW) - EV Accredited Issuing Body

    Pay Range:

    $54,230.45 - $100,875.88
