Norwalk Hospital is a 366-bed academic and community medical center serving Fairfield County, Connecticut, and the surrounding New York Metro area. We are a Level II Trauma Center and Joint Replacement Center of Excellence, and we provide advanced cancer, heart, and neuroscience care.

**
*Summary:*Assists in facilitating groups, preforming psychiatric assessments,providing treatment, and stabilization ofpatients for continued treatment. Strong collaboration withpsychiatry teamand other health professionals.
*Responsibilities:*
1.Facilitates group therapy, provides individual, family, crisis intervention, patient-centered supportand psycho-education. 20%
2. Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral. Identifies potential barriers, limitations & lack of coping abilities, adjustments, and functional status. Participates in patient care planning 20%
3. Conducts intakes with patients/family members to evaluate coping abilities, dysfunctional adjustments, maladaptive behaviors, functional status. 20%
4. Maintains knowledge of community resources and refers patients & families to agencies for supportive services & concrete resources (financial assistance; housing; transportation; medications) on regular & emergency basis. 10%
5. Completes hospital and state required documentation & other paperwork & forms, including insurance authorization requirements. 10%
6. Evaluates for child, elder abuse, and domestic violence and makes appropriate referrals. Acts as a consultant and/or educator to staff relative to assessment, criteria & referral. 10%
7. Acts as a liaison for the patients within community (local, state and private agencies). 10%
8. Fulfills all compliance responsibilities related to the position.
9. Performs other duties as assigned.
*Required:*
Master's Level Degree.
Minimum 3 years work related experience.
Current Licensed Clinical Social Worker (LCSW) or will accept Licensed Master Social Worker (LMSW). LCSW must be attained within 24 months from date of hire.
