POSITION SUMMARY
This position is accountable for the quality of rehabilitative services delivered by self and through the supervision of team members who are under their direction. This position utilizes specialized knowledge, judgment, and rehabilitation skills necessary to assess data and plan, provide and evaluate care. Manages personnel to ensure appropriate allocation of resources in providing quality patient care and appropriate rehabilitation care of patients in accordance with organizational policies, regulatory bodies, and leadership direction.
CORE FUNCTIONS
1. Provides, facilitates and supervises patient centered care and care coordination to ensure patient care needs are met while maintaining quality standards and regulatory compliance in a financially responsible manner. Performs work with minimal supervision. Assures efficient operations to meet customer needs. Internal customers include physicians, staff, and management. External customers include patients, families, and surrounding community.
2. Serves as a resource to patients, families, physicians, and staff by providing hands on care, clinical support, managerial support, interpreting policies, and facilitating care. Provides departmental leadership by demonstrating effective communication skills, problem solving, and proactive conflict resolution to internal and external customers in situations.
3. Hires, leads, mentors, develops, and evaluates staff in a manner that promotes engagement, productivity, and ensures competencies. Mentors staff to increase clinical skills, critical thinking, problem solving skills and promote retention. Conducts individual and team goal setting, performance planning and evaluation. Supports staff in adoption of change and participates in the development, interpretation, implementation and evaluation of process improvement and quality management activities.
4. Supervises and delivers care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
5. Ensures effective use of resources, equipment and supplies. Identifies and adjusts staffing levels needed to meet changing workload projections and provide safe, quality patient care. Ensures smooth and efficient patient care management. Promotes continuity of care by accurately and completely communicating to scheduling teams, other caregivers, and organization to service the system’s patients.
6. Leads, promotes, and participates in interdisciplinary patient care planning and education. Assesses appropriate level of care for patient population. Elevates utilization or care plan issues for patients under their supervision to ensure patient goals of care are met.7. Incumbents in this position may work independently under general supervision to fulfill responsibilities using specialized skills and knowledge. This position interacts with a wide variety of individuals including staff, co-workers, patients, physicians, visitors, regulatory agencies, surveyors, inspectors, contractors, and vendors. This position is facility based and has no budgetary responsibilities.
7. Formulates, implements, and supervises creation of a plan of care, including the discharge plan, through direct patient care, care coordination, delegation ad supervision utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care.
8. Ensures documentation, assessments, and care plans are timely and in accordance with policy. Assures that documentation reflects objective/subjective data, nursing interventions and patient's response to treatment as they progress towards expected goals and outcomes.
MINIMUM QUALIFICATIONS
Graduate of an accredited program in Occupational, Physical, Audiology or Speech Therapy required. Must hold a current license or certification in respective discipline.
A minimum 2 years of Home Health experience required.
PREFERRED QUALIFICATIONS
Additional management or supervision experience preferred.
Additional experience in Home Health a plus.
Additional related education and/or experience preferred.
