    Physical Therapist – Home Health Plattsburgh, NY (Clinton County)

     

    Are you a licensed Physical Therapist who thrives in a home-based care setting and wants to help patients rebuild strength, mobility, and independence in their own homes? HCR Home Care, a trusted provider of compassionate home health services, is looking for a Full-Time Physical Therapist to serve patients throughout Clinton County, NY, with a focus on Plattsburgh and surrounding communities.

    What You’ll Do

    + Conduct comprehensive evaluations using diagnostic tests for muscle, nerve, joint, and functional ability.

    + Develop detailed assessments and individualized plans of care with recommended treatment types and frequency.

    + Communicate with physicians regarding plan of care recommendations and necessary adjustments.

    + Document clinical notes and maintain accurate, up-to-date patient records.

    + Deliver therapeutic interventions to address impairments and maximize functional independence.

    + Educate patients and caregivers on treatment plans, goals, and appropriate use of durable medical equipment (e.g., canes, walkers, wheelchairs).

    + Participate in team meetings, case conferences, and professional development sessions.

    + Ensure compliance with safety protocols and maintain therapy equipment.

    What You Bring

    + Graduation from an accredited Physical Therapy program.

    + Current, valid Physical Therapy license issued by the NYS Department of Education.

    + Minimum of one year practical experience as a licensed Physical Therapist.

    Work Environment + Physical Requirements

    + Work primarily in patients’ homes throughout Clinton County, NY.

    + Role involves medium physical effort — walking, standing, sitting, stooping, kneeling, and repetitive motion — caring for patients across diverse home settings.
