RN Clinical Manager – Home Health | Rochester, NY (Monroe County)
Are you a seasoned Registered Nurse passionate about clinical excellence and team leadership?
HCR Home Care is seeking an RN Clinical Manager to oversee and elevate the delivery of skilled home health clinical services in Rochester, NY and Monroe County. In this critical leadership role, you will direct clinical operations, supervise nursing staff, ensure regulatory compliance, and support a high standard of patient-centered care across our home health team.
What You’ll Do
+ Lead day-to-day clinical operations and ensure patient care is executed according to plan of care and regulatory standards.
+ Supervise interdisciplinary nursing staff (RNs and LPNs) and mentor clinical performance.
+ Oversee clinical assessments, care delivery, and patient transitions in collaboration with physicians and departments.
+ Review complaint forms, quality improvement (QI) reports, and proactively address clinical and operational issues.
+ Conduct case conferences and ensure documentation accuracy.
+ Approve schedules, ensure productivity standards, and assist with staffing levels to meet clinical needs.
+ Support recruitment, retention, education, and performance evaluations for clinical staff.
+ Participate in community engagement and promote a culture of quality and safety.
What You Bring
+ Current RN license issued by the New York State Department of Education.
+ Bachelor's degree in nursing required.
+ Minimum of two years’ experience in certified home health care, including supervisory or management experience.
+ Experience in quality improvement, care coordination, and staff development.
+ Strong leadership, communication, and problem-solving skills.
+ Valid NYS driver’s license, dependable transportation, and ability to travel as needed.

Work Environment
+ Primarily office-based with some outdoor exposure. Classified as light work (sitting most of the time with some standing and walking).
