    Physical Therapist (Home Health) – Full-Time

     

    Hudson Falls, NY (Warren & Washington County)

     

    Are you a licensed Physical Therapist who provides high-quality rehabilitative care in patients’ homes? HCR Home Care is seeking a Full-Time Physical Therapist to deliver skilled therapy services to patients throughout Warren and Washington Counties, centered in Hudson Falls, NY. In this role, you will evaluate patients, develop and implement individualized plans of care, and support functional improvement and independence in a home health setting.

    What You’ll Do

    + Conduct comprehensive evaluations using diagnostic muscle, nerve, joint, and functional ability tests and measures.

    + Prepare patient assessments and individualized written plans of care including recommended treatment types and frequency.

    + Communicate care recommendations and modifications to physicians as needed.

    + Deliver therapeutic interventions aimed at maximizing patients’ functional independence and mobility.

    + Document clinical notes and maintain accurate, timely records.

    + Educate patients and caregivers on treatment goals and safe use of assistive equipment (e.g., canes, walkers).

    + Participate in staff meetings, case conferences, and professional development.

    + Ensure safety protocols and proper maintenance of equipment.

    What You Bring

    + Graduate of an accredited Physical Therapy program.

    + Current, valid New York State Physical Therapy license.

    + At least one year of practical experience as a licensed Physical Therapist.

    Work Environment

    Work primarily in patients’ homes across Warren and Washington Counties, often exposed to outdoor conditions. This role is classified as medium work, requiring standing, walking, sitting, and occasional lifting up to 50 pounds.
