Anoka Metro Regional Treatment Center (AMRTC) is seeking talented, mission driven, and person-centered Social Work Support Coordinator to join our team. A Social Work Support Coordinator promotes a timely return to the community through the development of a comprehensive discharge and aftercare plan, incorporating utilization management principles, risk management principles, and continuity of care. Care, treatment, and services are provided through the assessment of patient needs, development and coordination of care plans, and ongoing evaluation of service effectiveness. The employee works collaboratively with treatment team members and provides licensed social work case management coverage, including assistance with referrals for aftercare housing, psychiatric and other treatment services, discharge documentation, patient communication, transportation coordination, and other case management activities as needed.

Minimum Qualifications

To qualify, applicants must have:

+ Bachelor's degree in Social Work

+ Licensure (LSW) in the State of Minnesota

Preferred Qualifications

+ One (1) year of professional experience working with individuals with serious and persistent mentalillness (SPMI)

+ Knowledge of behavior modification principles sufficient to participate in the planning and designing ofindividual treatment programs for assigned clients.
