The Arc Oneida-Lewis, a pioneer in championing individuals with disabilities, is seeking a dedicated Licensed Practical Nurse (LPN) to support the health and well-being of individuals in our residential and day programs. This role provides hands-on nursing care while working closely with a Registered Nurse to ensure high-quality, person-centered medical supports.

ABOUT THE ROLE:
Under the direction of a Registered Nurse, the Licensed Practical Nurse assists with the oversight and coordination of healthcare services for individuals supported in residential and day programs. The LPN helps implement medical policies and procedures, provides direct nursing care, and supports staff in meeting the ongoing health and medical needs of the people we support, in accordance with agency, OPWDD/OMRDD, and regulatory standards.
Responsibilities:
+ Provide nursing services and direct care, including lifting assistance as needed, under the supervision of a Registered Nurse
+ Assist in overseeing the daily health and routine medical management of individuals, including basic therapeutic and preventative nursing procedures
+ Follow up on health-related concerns with staff, families, and healthcare providers as directed
+ Support individuals and staff during complex medical appointments, emergency room visits, and hospital stays
+ Assist with nursing assessments, summaries, and maintenance of medical records
+ Review annual physicals and medical reports and follow up on recommendations from physicians or the Registered Nurse
+ Assist with the storage, administration, and monitoring of medications for effectiveness
+ Maintain current, accurate health records for each individual
+ Obtain vital signs and weights according to prescribed schedules
+ Assist in monitoring personal hygiene, nutrition, and dietary needs; review menus for nutritional compliance
+ Support infection control practices and monitor communicable diseases
+ Provide guidance and instruction to staff and individuals related to health, safety, diet, and wellness
+ Administer medications as assigned and complete all required documentation
+ Participate in ISP meetings, staff meetings, and committees as assigned
+ Promote and adhere to agency values, Corporate Compliance, HIPAA, and safety policies
+ Maintain confidentiality and protect the rights, health, and well-being of the people we support
+ Attend and complete all required trainings
+ Adhere to scheduled hours and submit accurate timesheets in accordance with payroll deadlines
Qualifications:
+ Valid, current New York State Licensed Practical Nurse (LPN) license required
+ Specialized training or experience supporting individuals with developmental disabilities preferred
+ Valid driver's license with the ability to travel between program sites
+ Ability to work flexible schedules, including evenings and weekends as needed
+ Strong communication skills and ability to work independently and as part of a team
+ Ability to lift up to 50 pounds and perform physical aspects of the role
+ Corrected vision and hearing within normal range
Must obtain and maintain:
+ CPR and First Aid Certification
