Job Title: Registered Nurse (RN) – Start of CareLocation: Suffolk & Nassau County, NYEmployment Type: Full-Time, Part-Time, or Per DiemJob Description:The Start of Care Nurse plays a critical role in initiating home health services for new patients. You will perform comprehensive assessments and develop individualized care plans to ensure continuity and quality of care.About Always Compassionate Health:Always Compassionate Health is a trusted provider of home and community-based healthcare services across New York. Join our team and help support students in local schools with essential medical care.

Pay Rates:

+ Initial Start of Care: $150 per visit

+ Follow-Up Visit: $90 per visit

+ 90-Day Post-Operation Visit: $60 per visit

+ Aide Evaluation Visit: $40 per visit

Responsibilities:

+ Conduct initial patient assessments and complete all required documentation.

+ Develop and implement individualized care plans in compliance with regulatory standards.

+ Educate patients and families on care procedures, medications, and health management.

+ Coordinate with interdisciplinary teams to ensure seamless transitions and ongoing care.

+ Maintain accurate records and ensure compliance with agency and state guidelines.

Qualifications:

+ Current and valid RN license in New York State.

+ Strong knowledge of OASIS documentation and regulatory requirements.

+ Excellent organizational and communication skills.

+ Ability to travel to patient homes within assigned territory.
