Primary Function/General Purpose of Position

The Supervisor, Community Health oversees and supports a community health team, ensuring community health programs, outreach and support activities run smoothly. This role works with Community Health leaders, as well as internal and external partners to ensure we meet the needs of community members and meets all regulatory and organizational requirements.

Essential Job Functions

+ •Provides day-to-day guidance, support, and oversight to community health team members. Tracks team members’ workload and performance, provides feedback, and addresses issues or challenges.

+ Supports implementation of community health services, policies, and procedures, including providing training and skill-building to teams as needed.

+ Participates in community health initiatives, including but not limited to outreach, community engagement, education and direct client support activities.

+ Provides input into continuous improvement of community health programs, policies, and procedures. • Maintains partnerships with local leaders, including representing the organization in the community and speaking with local groups on community health matters.

+ May support strategic planning and participate in grant writing as it relates to the work or development of new programs.

Education

Associates or Bachelor's degree in social work, public health, Community Health, Public Policy, or relevant field (preferred)

Work Experience

+ 2 years of experience in an acute care or community outreach setting (required)

+ 1 year of leadership experience (preferred)

+ Bilingual based on market needs (preferred)
