Job Description:
+ Performs behavioral health intakes, psycho-social assessments, care plans, and psychotherapy with patients and families coping with behavioral health disorders.
+ Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.
+ Collaborates with the multi-disciplinary team in establishing and documenting psycho-social determinants of health to attain safe discharges and avoid emergency hospital admissions.
+ Serves as a consultant and educator to the multi-disciplinary team regarding psycho-social components of illness and hospitalization.
+ Maintains professional competency by participating in continuing education and appropriate learning experiences, both internally and externally.
Knowledge, Skills, and Abilities:
• Excellent interpersonal and written communication skills
• Ability to act in an autonomous, self-directed manner while maintaining the ability to collaborate with other members of the team
• Excellent communication, negotiation and advocacy skills with patients, families, team members, health care team and community resources
• Working knowledge of community and treatment resources based on age and clinical need
• Critical-thinking/problem-solving skills
• Ability to utilize in-house and external resources
• Flexibility in prioritization and effective organizational skills
• Ability to utilize stress management techniques effectively
Education:
• Master's Degree
Field of Study:
• in Social Work or in Mental Health Counseling
Work Experience:
• 2+ experience in clinical social work
Additional Information:
• N/A
Licenses and Certifications:
• Clinical Social Worker License (LCSW)
• Basic Life Support - CPR Cert (BLS)
• Cardiopulmonary Resuscitation (CPR)
• Licensed Mental Health Counselor (LMHC)

$26.08 - $48.50
