    **Work Type:** Full Time (Total FTE between 0.9 and 1.0)

    **Shift:** Shift 1

    **Work Schedule:** 8 Hr (9:00:00 AM - 5:00:00 PM)

    **Pay Range:** $46.06 - $74.32 per hour

    Summary:

    The Clinical Nurse Navigator will provide care coordination, follow up and support to patients in an effort to improve their breast health outcomes. This position will be shared across low-resource facilities with the navigator working in the Task Force office. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

    Required Job Qualifications:

    •Bachelor's degree.

    •3-5 years of experience as an RN.

    •Experience conducting breast cancer related outreach and education.

    •Strong communication, organizational and interpersonal skills with the ability to effectively work with a diverse group of individuals.

    •Strong presentation skills.

    •Proficiency with Microsoft Office Suite and Access.

    •Data entry skills with attention to details.

    •High degree of accountability and reliability.

    •Ability to adaptable to change and interact with a variety of providers with different protocols and care processes.

    •Ability to work independently as well as to integrate as part of a team and part of clinic flow.

    •Cultural competency expressed via experience and interest in working with (culturally, economically and educationally) diverse populations, along with ability to work in low resource environments.

    •Must be a resourceful and resilient, innovative problem solver and motivated learner. Friendly, cooperative, passionate and compassionate.

    •Must be respectful of each individual's right to participate in health care services and decision-making.

    •Respects individuals' confidentiality.

    •Ability to attend some evening and weekend community events and meetings.

    Preferred Job Qualifications:

    •Knowledge of breast cancer and breast imaging.

    •BSN preferred, and current RN licensure in the State of Illinois.

    Responsibilities:

    • Patient Navigation Works with the project director to develop protocols to receive abnormal mammography results from each participating site.

    • Reaches out to women with abnormal mammograms to assist them with follow-up.

    • Offers guidance and emotional support to patients regarding decisions on follow-up and help to complete diagnostic process for breast cancer.

    • Arranges appointments with patients, reviews test preparation procedures and assesses potential patient barriers to ensure the patient makes it to the diagnostic mammography and, as indicated, completes the breast cancer diagnostic process.

    • Contacts patients via phone calls, emails and letters to remind them about appointments, assist in scheduling/rescheduling appointments and to answer questions about breast cancer diagnostic imaging, biopsies and treatment.

    • Performs follow-up phone calls to patients after appointments as indicated.

    • Works with participating sites to develop mechanisms for the sites to share the patient data (for those requiring follow-up) in a manner that utilizes their existing data and optimizes their time and effort.

    • Completes all necessary electronic records and paperwork regarding patient interactions and appointments.

    • Community Outreach.

    • Develops and maintains relationships with community leaders, community-based organizations, faith-based organizations, health institutions, schools, daycares, etc.

    • Attends community-based events, including health fairs, support group meetings etc.

    • Assists with the planning and execution of breast cancer forums, special events and presentations to reach and educate the community about the Task Force and its community-based initiatives, including Project Share and other Task Force navigation programs.

    • Supports reporting and paperwork to support funding and grant requirements, including tracking patient metrics such as appointment no-shows and lost to follow up ratios.

    • Tracks all outreach and navigation activities and provides monthly reports.

    • Performs related duties as required.
