    Job Profile** **Summary

    These roles focus on providing high-quality patient care and promoting wellness. APCs work collaboratively with physicians and other healthcare professionals to diagnose and treat a variety of acute and chronic conditions. In addition, this role focuses on conducting comprehensive patient assessments, ordering and interpreting diagnostic tests, developing and implementing treatment plans for acute and chronic illness, providing education to patients and families about preventive care, treatment options, and self-care techniques, and evaluation of therapeutic response to the treatment plan and with adjustment of interventions developed as needed. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience. Typically responsible for: managing projects / processes, working independently with limited supervision, coaching and reviewing the work of lower level professionals, resolving difficult and sometimes complex problems.

     

    Job Overview

     

    Responsible for assisting the Clinical Nursing Director in the planning, delivery and management of direct patient care for an assigned unit/ group, utilizing the nursing process and adhering to the standards of nursing practice. Assists the Clinical Nursing Director in translating the philosophy, objectives, policies and procedures of the hospital and nursing service into effective action.

    Job Description

    Minimum Qualifications** **:

    1. Master’s degree in Nursing (MSN).

    2. Registered Nurse (RN) license.

    3. Advanced Practice Registered Nurse (APRN) license.

    4. Basic Life Support (BLS) certification.

    5. Three (3) years of clinical nursing experience in relevant area of nursing practice.

    6. Certifications (i.e. ACLS, PALS) relevant to the department and type of care being provided may be required.

    **Duties and Responsibilities** **:** The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

    1. Demonstrates clinical expertise, integrating nursing theory and practice. Employs evidence-based practice in decision-making and patient care.

    2. Utilizes the nursing process for the delivery and management of patient’s care across the continuum of care appropriate to the developmental age of the patient population.

    3. Collaborates with primary and associate nurses and other members of the health care team related to patient plan of care.

    4. Mentors the nursing staff in the primary nursing role utilizing the principles of the primary nursing model.

    5. Develops and monitors systems that promote and support primary nursing practice.

    6. Interacts with staff nurses for the purpose of assessing the care provided and the nurses’ capabilities to provide care in changing circumstances.

    7. Provides consultation to patients, family members and nursing staff as issues arise regarding the patient’s hospitalization or care experience.

    8. Utilizes the quality assurance and performance improvement process to assess the response to nursing care from patients, families and others.

    9. Uses clinical skills to provide ongoing assessment and evaluation of nursing care.

    10. Coordinates and directs patient care to ensure patients’ needs are met and hospital policy is followed.

    11. Maintains open communication with managers whose departments provide support services to the clinical area in order to ensure the appropriate level of service at all times.

    12. Evaluates and communicates to manager changing needs of the unit, including staffing requirements.

    13. Demonstrates the ability to be flexible, organized and function well under stressful situations.

    14. Collaborates with other members of the unit management team in developing practice guidelines, competencies and unit specific procedures.

    15. Communicates and consistently supports the philosophy and goals of the unit, department and hospital.

    16. Supports the orientation and preceptor programs.

    17. Assists in the development and implementation plan of performance improvement activities.

    18. Assists in developing mechanisms for data collection related to quality indicators.

    19. Promotes staff participation in unit and hospital committees/task forces.

    20. Delegates appropriately to others in accordance with job description and competency level.

    21. Supports the ongoing development of staff through coaching, mentoring and role modeling.

    22. Collaborates with other members of leadership team in providing appropriate educational opportunities for licensed and unlicensed personnel.

    23. Participates in and provides leadership to departmental and hospital committees as requested.

    24. Assists in the development of practice initiatives through unit-based group activities.

    25. Enhances own professional growth and development through participation in educational programs, keeping current with literature, in-service meetings, and workshops.

    Physical Requirements** **:

    1. Prolonged, extensive, or considerable standing / walking

    2. Lifts, positions, pushes and / or transfers patients

    3. Considerable reaching, stooping, bending, kneeling, crouching

    4. Regularly exposed to the risk of blood borne diseases

    5. Contact with patients under wide variety of circumstances

    6. Subject to varying and unpredictable situations

    7. Handles emergency and crisis situations

    8. Day / rotator or permanent off shift position schedule

    Skills & Abilities:

    1. Demonstrates leadership skills appropriate to position’s responsibilities.

    2. Excels at demonstrating the knowledge and skills necessary to provide care to patients in the relevant area of nursing practice.
