Position OverviewThe inpatient rehabilitation facility admission therapist/coordinator works with the inpatient rehabilitation admission team to coordinate patient referrals and admission to UHSH inpatient rehabilitation. As a member of the admission team, employee will participate in admission assessments, admission decisions, care conferencing/case review including but not limited to coordination/communication with patients/caregivers, physiatrist, referring providers, therapists, nursing and care coordinators. The admission therapist provide therapy evaluation and treatment services for hospital-based patient care to support the needs of the inpatient rehabilitation program and/or the needs of the overall UHSH therapy department as needed. The therapist/coordinator will maintain specific service delivery standards which address commitment to the UHS Values; Compassion, Trust, Respect, Teamwork and Innovation. Additionally, the admission therapist/coordinator will support IRF program development through networking/marketing efforts to promote community awareness and increase market share.

Responsibilities:
+ Coordinates referral development and pre-admission screening process for potential inpatient rehabilitation patient candidates.
+ Performs clinical assessment of patients referred for potential admission to inpatient rehabilitation facility (IRF) to determine whether admission to IRF setting is recommended through:
+ Review of medical record(s)
+ Communication/collaboration with members of acute interdisciplinary care team, patient, & family/caregiver(s)
+ Utilization of therapeutic evaluation skills as needed
+ Reviews referrals with rehabilitation physician(s) to ensure appropriateness and to determine readiness for admission to IRF.
+ Facilitates transition to IRF setting including:
+ Completion of required Pre-Admission Screen and associated documentation
+ Insurance verification, seeking prior authorization where applicable
+ Hand off communication with IRF treatment team.
+ Acts as a patient advocate for post- acute care needs by providing education to patient/family regarding IRF admission criteria and by updating patient, family & care management team throughout the referral, acute discharge and IRF admission process.
+ Administers routine physical therapy, occupational therapy or speech/language pathology services (as applicable to discipline); effectively performing the essential functions of all other PT, OT or SLP clinicians.
+ Effectively and efficiently balances time and attention between clinical care and admissions responsibilities as needed and directed.
+ Collects, maintains and analyzes applicable data in order ensure quality and promote continuous process improvement within the admissions process. Shares and reports data with program team and program leader(s).
+ Establishes and monitors referral source information, providing education and marketing as needed to maintain and/or grow market share.
+ Collaborates with other UHS, Inc. organizations and community health care agencies in planning, implementing, and evaluating post-acute care health care delivery services.
+ Maintains an active role in the community and region as a representative of UHS Hospitals to market the inpatient rehabilitation program.
+ Understands and articulates reimbursement/funding strategies. Identifies and facilitates appropriate alternative levels of care or strategies to assure quality while decreasing costs.
+ Will perform additional responsibilities, as requested
Position Qualifications:
Minimum Required:
+ A graduate of an accredited school of Physical Therapy, Occupational Therapy, or Speech Language Pathology.
+ Three (3) years of clinical experience
+ Current New York State Licensed & Registered Physical Therapist, Occupational Therapist, or Speech Language Pathologist.
+ CPR within 3 months of hire
Preferred:
+ Previous experience in Inpatient Rehabilitation
+ Previous marketing/community networking/relations experience and/or similar experience within interdisciplinary clinical care team(s) where advocacy, education and networking skills were required.
+ Superior communication & referral building skills
