    We’re actively seeking a driven and compassionate Advanced Practice Provider (APP) to join our Rheumatology Outpatient Clinic at the Upstate Health Care Center at 90 Presidential Plaza.     + Deliver comprehensive, patient-centered outpatient Rheumatology care at the top of your license.

    + Partner closely with patients, families, primary care teams, and specialists to create meaningful, long-lasting health outcomes.

    + Evaluate patients, order and interpret diagnostic studies, and coordinate referrals and therapeutic services with confidence and autonomy.

    + Build powerful relationships within a highly collaborative, multidisciplinary care team.

    Minimum Qualifications:

    + Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program) or will be eligible by time of appointment.

    + NYS Licensed/Registered NP or PA.

    + Current National Certification consistent to the clinical practice.

    + NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.

    + CPR-BLS Certification required at the time of appointment.

    + Must successfully pass pre-employment and credentialing clearances.

    Preferred Qualifications:

    + 1or more years of experience in Primary care or Rheumatology Preferred

    Work Days:

    + Monday – Friday, 0800-1630; No weekends or Calls
