    Job Description

    A client of Insight Global operates a senior‑care program in Kokomo that helps adults 55+ remain safely at home through coordinated medical, therapeutic, and social support services, emphasizing whole‑person care, community impact, and strong continuity of services while offering meaningful work and opportunities for professional growth. They are seeking a Physical Therapist (PT) to join their interdisciplinary team, providing on‑site and in‑home therapy for older adults with a strong focus on collaboration, communication, and participant‑centered care; full‑time hours are Monday–Friday, 8:00 AM–4:30 PM, with part‑time options available.

    Responsibilities:

    - Perform evaluations and provide therapy on‑site and in participant homes

     

    - Complete required clinical documentation

     

    - Create and adjust individualized treatment plans

     

    - Collaborate with the care team during daily meetings

     

    - Recommend and coordinate necessary equipment

     

    - Educate participants, caregivers, and staff

     

    - Maintain a safe and compliant treatment environment

     

    - Support admissions and quality improvement effort

    Skills and Requirements

     

    - Doctor of Physical Therapy (DPT) from an APTA‑accredited program

     

    - Active state licensure as a Registered Physical Therapist

     

    - Minimum 2 years of Physical Therapy experience

     

    - Experience working with geriatric populations

     

    - Valid driver’s license, reliable transportation, and current insurance

     

    - Ability to travel to participant homes, nursing facilities, and clinic sites

     

    - Strong written and verbal communication skills

     

    - Proficiency with computers, Microsoft Office, and electronic medical records

    - Willingness to complete:

    - Drug screening

     

    - Background check

     

    - TB test - In‑home health experience

     

    - Familiarity with regulations related to senior‑care programs

     

    - Strong teamwork and collaboration skills

     

    - Ability to manage multiple priorities and deliver high‑quality outcomes

     

    - Commitment to ethical practice and ongoing professional development
