    What You’ll Do:

    Partner with pediatricians, nurses, and therapists to support comprehensive patient care

     

    Empower families and caregivers by connecting them with vital services and support networks

     

    Address social determinants of health and remove barriers to care for children in need

    REQUIRED QUALIFICATIONS

    Education: Master’s Degree in Social Work

    Skills:

    • Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills

    • Ability to work in self-directed environment with attention to detail, follow through, and has a high level of computer literacy.

    • History of positive rapport with patients, families, physicians, and interdisciplinary team

     

    License: Active LISW license

    PREFERRED QUALIFICATIONS

    Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty is preferred.
