    About the Role:

    Do you have a passion for supporting the health and well-being of others? Are you committed to social justice and ensuring access to quality healthcare for all? If so, Callen-Lorde wants to hear from you! We are seeking a Patient Navigator to join our dedicated team and play a vital role in supporting the needs of our patients living with HIV.

    As a Patient Navigator, your responsibilities will include:

    + Act as a patient advocate, ensuring patients' biopsychosocial needs are identified, addressed, and connected to appropriate services.

    + Provide field-based case management and adherence support services to HIV-positive patients experiencing behavioral challenges, including substance use disorders, that impact their ability to consistently engage in HIV care.

    + Work collaboratively with care coordinators, medical providers, and behavioral health professionals to facilitate patient retention and adherence to primary medical care.

    + Assist patients in navigating the healthcare system, overcoming barriers to care, and connecting them with essential resources.

    + Advocate for patients' rights and ensure they receive culturally competent care.

    + Document patient interactions and progress notes to ensure continuity of care.

    + Facilitate support groups to promote engagement, education, and adherence to HIV care and treatment.

    Work Schedule:

    This is a full-time position with evening hours.

    Qualifications

    + College degree (BA/BS) in health/social services or related field required.

    + Experience in social services, case management and/or entitlements work.

    + Familiarity with LGBTQIA, HIV and health-related issues and community resources.

    + Strong computer competency, including knowledge of Electronic Medical Records, Microsoft Office Programs, and internet research savvy.

    + Strong customer service orientation with an understanding of patient-led treatment.

    + Experience working with individuals with a spectrum of chronic illness conditions preferred.

    + Bilingual (Spanish/English) preferred.
