EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Certified Recovery Peer Advocate to join the Suffolk Transitional Case Management Substance Use Disorder program.

     

    Salary: $25

     

    Schedule:8AM-4PM, Monday- Friday

     

    Reports to: Program Director

    What are the key Responsibilities:

    Under the supervision of the Program Director, the role of the Certified Recovery Peer Advocate is to provide peer support, advocacy and linkage services to individuals enrolled in the Transitional Case Management-SUD program. The CRPA will provide a bridge between the TCM-SUD case managers, program participants and other community organizations, and support those enrolled in the TCM-SUD program. The successful candidate must be a self-starter and be willing to use their lived experience to help others who are seeking recovery from substance use issues. The CRPA will work as a fully integrated team member providing culturally responsive peer recovery support services.

    Principal Duties & Responsibilities:

    + Assist participants in the development of a personalized self-directed recovery plan.

    + Assist assigned case managers to link participants to formal recovery supports by providing information on existing community supports and services.

    + Accompany participants to appointments (court, medical, housing, etc.) as needed.

    + Document in the Electronic Health record (EHR) services provided to participants and maintain all aspects of documentation in accordance to regulatory requirements.

    + Provide non-clinical crisis support to program participants, especially after periods of hospitalization or incarceration.

    + Engage with individuals immediately upon their release from incarceration, and with those who have become disengaged with case management services and outside community treatment, especially after hospitalization or incarceration.

    + Share information with participants about benefits, entitlements and other support.

    + Model professionalism by adhering to ethical principles.

    + Participate in ongoing training to enhance professional development;

    + Educate program participants about various pathways to recovery.

    + Adhere to the treatment philosophy and mission of EAC Network.

    What key knowledge, skills, and abilities are required to excel in this role:

    + NYS Certified Recovery Peer Advocate (CRPA) or CRPA – Provisional. Successfully completed 46 hours of required training (30-hours recovery coach training and 16-hours of peer ethics).

    + Minimum of High School Diploma or educational equivalent required.

    + Working as a Recovery Coach or Provisional CRPA.

    + Lived experience in substance use and have knowledge of the criminal justice system.

    + Good written and verbal communication.

    + Must possess computer skills and the ability to navigate the electronic health record (EHR).

    + Exercise good judgment regarding privacy and confidentiality issues.

    + Knowledge of substance use disorder recovery process.

    + Able to work with diverse populations (ethnicity, sexual orientation, socioeconomic status, etc.).

    + Demonstrate commitment to providing quality services and adhering to outcome driven performance measures.

    + Understand the therapeutic benefit of Medication Assisted Treatment (MAT).
