    Job Description

    JOB FUNCTION:

    As the WellLife Network Children’s SPOA Behavioral Health Outreach Worker, the candidate is responsible for reviewing applications received by the Suffolk County Children’s Single Point of Access, a centralized intake and monitoring process for referral of high intensity mental health services for children/youth who have been diagnosed with a serious mental illness and whose illness interferes with their ability to function in the community, home and school. Work is performed under the supervision of the Children’s Services Social Worker (LGU) under the administrative oversight of the Suffolk County DOH, Division of Community Mental Hygiene Services Director of Children’s Services.

     

    + Obtain and review pertinent documentation for referral completeness;

    + Contact referral source and other key individuals to gather additional information as necessary;

    + Responsible for direct client contact with children and their families to assist with navigating multiple children serving systems, referral processing, program recommendations, and linkage;

    + Maintains steady and effective communication with Program Directors and staff;

    + Participate effectively in multidisciplinary meetings;

    + Knowledge of waitlists to assist in monitoring and outreach;

    + Assist families and providers navigate multiple children serving systems;

    + Handle telephone inquiries requesting information about CSPOA programs and services;

    + Make presentations at various outside agencies on an as needed basis

    Qualifications

    QUALIFICATIONS:BACHELOR’S DEGREE IN BEHAVIORAL HEALTH OR RELATED HEALTH/HUMAN SERVICES FIELD AND 1 YEAR EXPERIENCE. WORKING KNOWLEDGE OF CHILDREN’S BEHAVIORAL HEALTH SYSTEM, A PLUS.

    COMPETENCIES:

    + Excellent communication and interpersonal skills, professional demeanor, and team orientation;

    + Excellent organizational skills; flexible and adaptable to change;

    + Efficient time management with accuracy and attention to detail;

    + Proficiency in Microsoft Office, including Work and Excel, and Outlook;

    + Utilize the CSPOA database/EMR in a proficient manner
