    Program Description:

    Graham’s Care Coordination team plays a vital role in supporting the health and well-being of children and youth in foster care. Our work ensures that every child—no matter their background or circumstance—receives coordinated, high-quality medical, behavioral, and mental health care. We work closely with caregivers, service providers, and city and state partners to break down barriers and build up systems of support, all grounded in trauma-informed and family-centered care.

    Principal Objective/Role Overview:

    TheDirector of Care Coordinationleads a dynamic, mission-driven team of clinical and nurse care coordinators, supervisors, and administrative staff focused on ensuring every child in our care receives the services they need—from intake through permanency. This role is all aboutworking seamlessly across systemswhile supporting staff to do their best, most impactful work. The Director brings together strategic oversight, hands-on leadership, and a strong understanding of child welfare and medical/behavioral health systems.

     

    This is a role for someone who thrives onproblem-solving, cross-functional collaboration, and driving meaningful outcomesfor children and families.

    Job Responsibilities:

    Care Coordination & Clinical Oversight

     

    + Oversee and support care plans that are responsive, up-to-date, and aligned with each child’s needs.

    + Ensure timely referrals and follow-up for medical, behavioral health, and specialty services.

    + Collaborate with foster and birth parents, case planners, and providers to strengthen caregiver capacity and promote child well-being.

    + Respond immediately to alerts (e.g., ER, inpatient) and coordinate with hospitals and providers for discharge planning.

    + Partner with the Health Homes team to coordinate services for high-risk youth and monitor eligibility for complex care supports like SED, chronic conditions, and trauma.

     

    Compliance, Documentation & Quality Improvement

     

    + Ensure care coordination practices align with ACS, OMH, DOHMH, Medicaid, and 29i standards.

    + Track, review, and ensure accuracy of all documentation across systems (Connections, eCMS, Aura, SharePoint, etc.).

    + Conduct quality assurance reviews to ensure excellent service delivery and readiness for audits.

    + Use data dashboards and reports to track performance indicators such as acuity, show rates, and care outcomes—and translate insights into action.

     

    Team Leadership & Development

     

    + Supervise and support a team of care coordinators using the Kadushin model (administrative, clinical, and task supervision).

    + Provide feedback, coaching, and professional development opportunities that foster growth and accountability.

    + Facilitate trainings and team meetings on clinical assessments, documentation, and family engagement strategies.

    + Model collaborative leadership and help staff navigate difficult conversations, problem-solving, and cross-team coordination.

     

    Strategic Collaboration & Agency Engagement

     

    + Partner with other Graham departments and external partners to ensure coordinated and holistic service planning.

    + Represent Care Coordination in internal and external meetings (e.g., VOLAG, ACS, Health Homes, etc.).

    + Stay informed of evolving policy and regulatory changes—particularly in preparation for 29i licensure—and ensure the team is ready to adapt.

    Qualifications

    Position Qualifications:

    + LMSW/LCSW/LMHC, RN, MPH

    + Experience in child welfare, foster care, and/or health care coordination.

    + Strong leadership and team development skills; experience supervising multidisciplinary teams is a plus.

    + Deep understanding of trauma-informed care, family systems, and child/adolescent development.

    + Proficiency with electronic health records, data tracking systems, and Microsoft Office Suite.

    + Ability to manage competing priorities, think critically, and drive results in a fast-paced environment.

    + Excellent written and verbal communication skills.

    + Bilingual in Spanish a plus.

    Salary & Compensation:

    + Base Salary or Hourly Rate: $90,000.00/year

    + FLSAStatus: Exempt
