    Job Details:

    Directly provides counseling, case management and other chemical dependency services in meeting the social emotional needs of the detox/rehab patients and their family members.

    Responsibilities:

    + Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively.

    + Provide evaluations and psychosocial assessments for patients in collaboration with the td team.

    + Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients.

    + Prepares and keeps a current case record for each assigned patient according to HealthAllianceHospital, OASAS, and JC standards.

    + Participate as a team member in conferences, treatment planning on behalf of patient.

    + Attend in-service training and keeps professionally updated through reading and attending seminars.

    + Provide lectures, workshop/training for patients and staff as assigned.

    + Strictly adheres to all of the confidentiality requirements imposed by law and professional conduct.

    + Observes adherence to current policies and a resource to director for program recommendations.

    + Perform any other duties as assigned.

    Requirements/Qualifications:

    Experience:

    + Minimum one (1) year experience providing mental health treatment preferred.

    + Experience using one or more evidence-based treatment approaches preferred.

    + Experience working with individuals with SUD preferred.

    Education:

    + Masters of Social Work or Mental Health Counseling from an accredited program.

    Licenses / Certifications:

    + Current NYS Masters Level licensed QHP or licensure eligible with limited permit.
