    This role involves providing coordinated care to patients/clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The Physical Therapist will plan, organize, and conduct physical therapy treatment programs based on medical referrals and their evaluations, ensuring effective coordination of care.

    Requirements

    Key Responsibilities:

    + Develop and implement individualized physical therapy treatment plans.

    + Evaluate and document patient/client progress, adjusting treatments as necessary.

    + Coordinate care with other healthcare professionals to ensure comprehensive treatment.

    + Educate patients/clients and their families on treatment plans, home exercises, and preventative care.

    + Maintain accurate and timely patient records in compliance with company and regulatory standards.

    + Utilize durable medical equipment as needed and provide training to patients/clients.

    + Ensure patient safety and comfort during therapy sessions.

    + Participate in continuous professional development and stay updated with the latest therapy practices.

    Job Qualifications:

    + Education:Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.

    + Licensure:Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, or meets one of the requirements listed in 42 CFR §484.115(h).

    + Experience:At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.

    + Skills:Proficiency in therapy skills as defined by accepted standards of practice. Strong interpersonal skills. Current CPR certification. Knowledge of durable medical equipment.

    + Transportation:Reliable transportation, a current Driver’s License, and valid and current auto liability insurance.

    Environmental and Working Conditions:

    + Work in healthcare facilities or patients’/clients’ homes in various conditions.

    + Possible exposure to blood, bodily fluids, and infectious diseases.

    + Ability to work a flexible schedule and travel locally.

    + Some exposure to unpleasant weather conditions.

    Physical and Mental Effort:

    + Ability to perform heavy lifting, bending, pulling, pushing, and prolonged standing and walking.

    + Must be able to address patient/client and family psychosocial needs effectively.

    + Requires good hand-eye coordination and manual dexterity.

    $40.00 - $55.00
