    BayCare is currently in search of our newest **TOC Clinical Pharmacist** who is passionate about providing outstanding customer service to our community.

     

    **Clinical Pharmacist Responsibilities** :

     

    The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.

    + **Facility:** BayCare System Office

    + **Status:** PRN

    + **Shift:** 8:00AM - 4:30PM

    + **Weekend Work:** Occasional

    + **On Call:** No

    Certifications and Licensure:

    + **Required:** Pharmacist - State of FL

    + **Preferred:** ASHP (Pharmacist)

    Education:

    + **Required:** Bachelor's Degree; PharmD preferred

    Experience:

    + **Preferred:** PGY-1 residency; PGY-2 residency
