Job Summary and Qualifications
The Disease Management Coordinator provides leadership and promotes excellence in patient care of neurologic and other disease conditions in the Emergency Department, ICU, Medical Surgical and Rehabilitation Units, along with other entities within the healthcare team. This position is responsible for planning, development, and implementation of program goals and operations including Primary Stroke Center Designation. The Disease Management Coordinator also coordinates education of staff and physicians, oversees the ongoing quality improvement process to improve patient care. This position ensures compliance with the accreditation standards and collects and reports data to external agencies (as required) and internal committees.
What you will do in this position:
• Perform daily concurrent chart review to ensure compliance with evidence-based program measures for Stroke.
• Ensure all data reporting timelines are met (IRR, normalization, etc).
• Collaborate with clinical staff, physicians, and leaders to ensure evidence-based measures are met in a timely manner.
• Implement and revises the required policies, procedures, and protocols for Stroke Services.
• Perform patient/family education on Stroke Care (as needed).
• Serve as a resource to physicians, nurses, educators, and leaders on the evidence-based measures for Stroke.
• Identify educational and process improvement opportunities that support compliance with program measures.
• Coordinate with nursing staff and hospital leaders to establish and implement standardized, evidence-based processes/protocols.
• Analyze trends, and reports data to various hospital and regional committees (Quality Committee, Stroke Committee, CATRAC Subcommittees, etc.).
• Facilitate the hospital Stroke Committee meetings.
• Represent the hospital at a local, regional, and national level related to best practices for Stroke patients.
• Maintain knowledge and understanding of Stroke accreditation requirements.
• Compile and submit accreditation program application, performance improvement plan, and measures of success as required.
• Perform tracers to evaluate hospital compliance with regulatory standards.
• Attend and actively participates in monthly staff meetings, and attends called departmental meetings when necessary.
• Provide community education opportunities to promote health awareness in Stroke prevention.
• Attend and participates in facility committees, employee forums, and departmental meetings as requested.
• Actively utilize and comply with facility principles of good communication and customer service standards, including use of AIDET and KWAKT as developed by the department.
• Maintain compliance with required licensure, ethics and compliance training, and mandatory education as required.
• Other duties as assigned.
• Practice and adhere to the “Code of Conduct” and “Mission and Value Statement.”
Requirements
Experience
• 2+ years of experience in healthcare
• 1 year experience with stroke medicine
Education
• Bachelor of Science in Nursing (BSN) from accredited Nursing program – required
License and Certifications
• Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License Texas license must be obtained within 90 days of hire
