    Title: Senior Director, HIV Care Coordination Services

     

    Department: Health Care Coordination

     

    Reports To: Vice President, Programs, Care Coordination

     

    Position Status: Full Time

     

    FLSA Status: Exempt

     

    Pay Rate & Benefits: The pay range for this role is generally$70,000-76,000 annually, commensurate with relevant experience and qualifications and in alignment with internal equity.     Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low-income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.

    POSITION SUMMARY:

    The Director of HIV Care Coordination Services provides oversight and management of the HIV care coordination grant funded programs, staffing, and initiatives across the Department. It is the Sr. Director’s responsibility to ensure that each contract is understood, implemented, documented, and reported on to its respective funder, with all the activities and workers supported by each contract working collaboratively.

     

    The Care Coordination Program focuses on prompt linkage to HIV care, ongoing retention in care, optimal treatment adherence, viral load suppression and reduced HIV-related mortality. The Engagement and Support Services Program, another facet of this portfolio, assists people living with HIV/AIDS to improve overall health and establish self-management through case management, peer navigation, and health education services. These program elements are goal-driven and focus on improving medical outcomes, achieving viral suppression, and self-management skill development.

    DUTIES & RESPONSIBILITIES:

    + Oversee CHASI’s HIV services with focus on fidelity to program models and outcomes.

    + Provide management of the programs including monitoring, reporting, and quality assurance activities.

    + Monitor program performance and develop action plans to ensure that program targets and deliverables are achieved.

    + Provide directions of the day-to-day operations of the site and programs.

    + Manage program personnel, including collaborating with Human Resources on recruitment, orientation, ongoing supervision, evaluation, and corrective action, as needed.

    + Serve as a liaison between agency and program funders and other key players. Participate and represent CHASI in conferences, task forces, coalitions.

    + In collaboration with the division’s Vice President, develop annual work plans that clearly articulate measurable quantitative and qualitative goals and strategies that are coordinated with funder objectives.

    + Prepare monthly and as-needed reports for submission internally to management and externally to funders regarding combination prevention efforts.

    + Ensure compliance with agency, government, and funding source policies.

    + Ensure timely and accurate reporting of the programs and that they are meeting pre-determined deliverable targets.

    + In collaboration with the Department head, liaison with contract managers to collectively work toward program improvement.

    + Participate in regular supervision, CHASI’s Management Team meetings, strategic planning, workgroups, and committees.

    + Align all activities with CHASI’s mission, core values, and culture.

    + Other duties as assigned.

    Requirements

    QUALIFICATIONS:

    + Master’s degree with 4 years, Bachelor’s degree with 6 years, or high school diploma/equivalency with 8 years of experience overseeing or working with HIV related programs and clients, or other directly related experience, required.

    + At least 2 years of experience supervising staff and managing programs required.

    + Ability to work in a team setting and use effective leadership and communication skills.

    + Ability to work independently, establish priorities, and handle multiple tasks simultaneously.

    + Strong oral and written communication skills, including the ability to effectively present information and respond to questions from internal/external sources as well as the ability to relate effectively to clients, front line staff, middle, and executive management.

    + Ability to read, analyze, and interpret financial reports, service data, contracts, technical procedures, governmental regulations; able to write reports, P&P manuals, and professional correspondence. Able to respond quickly and effectively to improve program performance required.

    + Advanced computer skills, specifically Microsoft Office applications, including Excel and PowerPoint as well as internet navigation and ability to use and quickly learn new databases.

    + Ability to effectively present information and respond to questions from groups of clients, staff, managers, funders, policymakers, and the public.

    + Must live in a location that allows for the ability to respond to a crisis in person within 2 hours, as needed.
