Description of Duties:
This is a grant funded position. -Work with patients, families, peer advocates and mental health providers to develop comprehensive service plans for clients with mental health and substance use disorders. -Provide individual and group counseling- both on-site and remotely as needed-for clients regarding issues of chemical dependency, HIV and HCV prevention, and trauma; -Refer patients for wrap-around /recovery support services such as housing, child care, vocational, educational services; -Coordinate service plans with behavioral health service providers and community based organizations providing services to clients; -Contribute to patients care plan development by regular attendance at case conferences; participate in ongoing quality improvement activities; -Administer mental health and substance use screening tools and conduct patient interviews in accordance with SAMHSA protocols. -Record session notes and patient follow-up in the electronic health record .
Qualifications:
REQUIRED: New York State licensed social worker (LMSW/LCSW) or mental health counselor (LMHC); with 1-2 years' post-license experience providing solutions focused counseling to individuals with psychiatric disorders, substance abuse diagnosis (e.g. harm reduction and relapse prevention cognitive behavioral treatment models) and experience in the treatment of trauma. PREFERRED: CASAC or CASAC-T preferred. Previous training in Cognitive Behavioral Therapy, Motivational Interviewing and/or Seeking Safety strongly preferred. Experience in the provision of counseling to LGBTQ and gender non-conforming populations preferred. Previous experience providing counseling to individuals at risk or diagnosed with HIV preferred. Bilingual English/Spanish or English/Creole preferred. Must be proficient with Zoom, Microsoft Teams, Office Suite; and experience with an electronic health record (HER) preferred.
