Position Overview

As a Home Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall, ID area and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.

What You Can Expect in This Role

+ **Full-time Schedule** : Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts

+ **Manageable Caseloads** : Daily visit requirements are below the state and national average

+ **Compensation** : Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and its surrounding regions

+ **Additional Pay** : On-call pay and applicable differential pay

+ **Incentives** : Extra cost-of-living adjustment for McCall residents

+ **Professional Development** : Cross-training opportunities in hospice care

Qualifications

+ **Education** :Nursing Degree (ASN or BSN)

+ **Experience** : 2+ years of RN experience; Home Health or Acute Care experience preferred

+ **Licenses/Certifications** : Current RN license in the state of Idaho; Basic Life Support (BLS) certification

+ **Other Requirements** : Valid driver’s license and proof of auto insurance
