Salary Range: $24.00 - $31.00 Hourly

Role and Responsibilities

Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse.

Essential Functions

Under the direct supervision of a n RN Case Manager , and within the scope of practice for a L icensed P ractical N urse in our Transcultural team:

+ Provide health education according to the teaching plan established by the RN.

+ Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.

+ Observe signs and symptoms and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition , to the physician and RN Case Manager .

+ Document any services rendered to the patient, changes noted in the patient’s condition , and/or family and home situations.

+ Document communication with RN C ase M anager and physician.

+ Other duties as assigned.

Education Requirements

+ Associate's degree required .

+ Diploma as a Licensed Practical Nurse from an accredited program.

Qualifications and Requirements

+ Current, valid LPN license issued from NYS Department of Education.

+ Experience in home health care preferred.

+ Community health experience preferred.

+ Fluent in Spanish is required.

Work Environment

The LPN is primarily in a non-office setting and may be exposed to outdoor conditions.

The working conditions are classified as medium work:

+ Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
