    Job Description

    The mission of the Bureau of Tuberculosis Control (BTBC) is to prevent the spread of tuberculosis and to eliminate it as a public health problem in New York City. The goals to achieve this mission are twofold: 1) To identify all individuals with suspected or confirmed TB disease and ensure appropriate treatment; 2) ensure that individuals at high risk for progression from Infection to TB disease are identified and treated for TB infection and non-developed TB disease.

     

    Every TB patient in NYC is assigned to a Case Manager. Case Managers in the Immigrant and Refugee Unit specialize in reaching out to patients who have moved to NYC with a known risk factor for TB infection or disease. These Case Managers assist bureau and clinic staff in scheduling patients for clinic visits and documenting the outcomes of patient evaluations. This work is essential for protecting the heath of the individuals referred for screening, as well as their family members and contacts.

    DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

    Interview / re-interview and educate patients infected with (or at risk for) a communicable disease

     

    Refer patients to the appropriate diagnostic facility for medical examination and treatment

     

    Establish and maintain communication with co-workers, supervisors, managers, and community partners

     

    Report on the outcomes of clinical evaluations for TB among people who have been identified as at-risk for TB infection or disease

     

    Submit written reports including tracking changes in the number of individuals referred for TB screening, challenges and barriers associated with outreach efforts, and measures to minimize such challenges

     

    Present brief reports at quarterly team meetings

     

    Create and share with supervisor lists tracking progress for each patient assigned to them, to monitor progress and address issues in a timely manner

     

    Initiate collaborative efforts with team members to improve case management activities and provide feedback to supervisor and peers

     

    Represent supervisor in meetings and webinars when necessary

    PREFERRED SKILLS:

    Proficient in the use of Microsoft applications including Word, Excel, PowerPoint, and Outlook

     

    Excellent oral and written communication skills

     

    Strong attention to details and organizational skills

    Qualifications

    1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or

    2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or

    3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least one year of experience as described in "1" above.
