    Salary: $2800 / Week

     

    Start Date: ASAP

     

    Duration/Schedule Full time: M-F 8-5

     

    On-call/Callback: No call is needed

     

    Patient volume: 3+ patient visits daily

    Requirements:

    + Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home

    + Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license

    + Current and unrestricted DEA certificate

    + Ability to work without direct supervision and practice autonomously

    + 2 or more years of direct patient care required

    + Managed Care/IPA/Health Plan experience

    + Access to transportation, a valid driver's license, and car insurance

    + 2 years of direct patient care relating to polychronic conditions
