    About the Role

     

    As a **Physical Therapist – Home Health** , you’ll deliver personalized in-home care that helps patients regain strength, mobility, and independence. You’ll evaluate and treat clients with physical and emotional limitations in the comfort of their own homes—designing therapy plans tailored to their goals and environment.

    Key Responsibilities

    + Perform timely home evaluations and develop customized treatment plans based on physician referrals and patient needs.

    + Set functional, measurable goals and modify therapy based on patient progress.

    + Educate patients, families, and caregivers on therapy, safety, and exercises to enhance recovery and independence.

    + Collaborate with interdisciplinary teams and participate in case conferences to ensure whole-person care.

    + Maintain accurate documentation in compliance with home health standards and federal regulations.

    + Adhere to MercyOne’s safety and infection control policies.

    What You Need to Succeed

    + **Licensed Physical Therapist** in the state of Iowa.

    + **Master’s degree in Physical Therapy** (Doctorate preferred).

    + Valid **Iowa driver’s license** , insured vehicle, and reliable transportation.

    + **BLS certification** and Mandatory Reporter training required.

    + 0–2 years’ experience in physical therapy (home health experience preferred).

    + Strong communication, time management, and independent problem-solving skills.
