Sign on Bonus Available

Shift/Schedule: Mon-Fri from 8-5 p.m. 1 on call weekend per month. We are also hiring for PRN.

Responsibilities

+ Skilled Visits

+ Assists in development, review and revision of the plan of care.

+ Provides quality patient care set forth by the RN Case Manager's Care plan

+ Observes, records and reports reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician.

+ Provides ongoing instruction and supervision of client, family or other health team personnel in the plan of treatment.

Requirements:

+ Geriatric Home Health Experience

+ Texas Nursing License

+ Strong organizational and self-management skills

+ Strong and compassionate customer service skills

+ Valid driver’s license
