    At CMCS (Cardinal McCloskey Community Services), our mission is to protect, empower and promote independence for at-risk Children & Families and those with developmental disabilities through quality community-based services.

     

    As a Case Planner, you are responsible for the overall management of the provision of services to each child, foster boarding parent and biological family on his / her assigned caseload. This includes assessment of needs of each individual and the detailing and implementation of a treatment plan designed to effect the earliest possible discharge of each and every child to a permanent living situation. The Case Planner is also responsible for supportive coaching and guidance to the foster care program. Maintaining regular contact with the foster parent, at least monthly, in order to facilitate the child's adjustment to the foster home this includes sharing pertinent information about the child's past, interpreting problem behaviors, and offering suggestions around more effective parenting techniques. Responsible for court appearances and testifying in court related to assigned cases.

    Requirements:

    + BA / BSW and 2 years Human Services experience required.

    + Work experience in Child Welfare &/or Office of Mental Health System preferred

    + Computer literacy required

    + NYS driver's license preferred

    + Bilingual in Spanish preferred
