Harborview Medical Center - Mental Health and Addiction Services** has an outstanding opportunity for a **Mental Health Outreach Case Manager** **WORK SCHEDULE
+ 40 hours per week

+ Be a part of a dedicated team making a difference in our community **DEPARTMENT DESCRIPTION**
This position coordinates with the Harborview Mental Health and Addiction Services treatment teams, partner agencies and other members of our clients’ support systems who are working with people in need of psychiatric assessment and other mental health related support.
This position is responsible for implementing the goals of the program, which are to promote community stability for enrolled clients, engagement in mental health services and treatment planning as well as assistance in securing and maintaining housing, benefits and culturally relevant treatment to meet their needs, and to assist patients in developing meaningful and productive activities. The Outreach Mental Health Case Manager is expected to be able to work with a wide variety of patients.
JOB RESPONSIBILITIES
+ As part of a multidisciplinary treatment team work with patients and their social networks to assist in their recovery from mental illness and substance use disorders
+ Assess patient needs in clinic and in the community and determine interventions
+ Facilitate diversion from voluntary and involuntary psychiatric hospitalization when clinically appropriate
+ Coordinate medication services with team’s medical staff
+ Assist in the development and planning of groups relevant to the needs of clients
+ Prepare information requested by outside agencies such as DSHS, Social Security Administration, and Seattle Housing Authority
+ Participate in case reviews
+ Complete data logs, chart patient contact in the medical record
+ Collect data for evaluation and quality improvement **REQUIRMENTS**
+ A Master’s Degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work
+ OR Bachelor’s Degree in nursing or a behavioral science AND three years of experience in the direct delivery of professional mental health care
+ Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration
+ Licensed as a Mental Health Counselor, Social Worker, or Psychologist is preferred **ABOUT HARBORVIEW MEDICAL CENTER**
