    Position Overview:

    Maintains the integrity of the Electronic Health Record by assuring that transcribed reports are attached correctly, routed to appropriate location and corrected timely and accurately. Responsible for the overall Data Integrity, Quality of the Transcribed report, EMPI merges in the Electronic Health Record (EHR), legacy and on paper. Works in conjunction with the Shared Service Center.

     

    **Job Description** :

    + EMPI merges, Reconciles identification of documentation missing patient identifiers. Monitor and works from EMPI work queues and reports using EPIC.

    + Merges duplicate patient health records in EPIC , Legacy system and on paper. Maintains organizational goal of 2% or below. Updates and maintains history Combined link duplicate number file. Reports duplicate /registration error trends to Admitting Director for follow-up.

    + HIPAA Audits, Reviews accesses to the Electronic Health Record.

    + Processes transcription daily. Performs daily quality audit of transcribed reports on problem dictators by listening to audio file, and making corrections on report, as needed. Performs daily quality audit on transcribed reports for blanks, completing blanks or forwarding back to transcription company if undecipherable. Serves as a liaison between department and transcription company.

    + Navigates EMON, EPIC and Onbase. Works EMON pending queue daily. Ensure all corrections have been made. Distributes dictation through interface and other transcription related work queues in EHR and eScription.

    + Monitors eScription fax distribution reports and redistributes if failed. Review daily eScription new contact alert findings and trends Follow up with physician when needed, forwards concerns to manager.

    + Reports all transcription system outages to transcription company and manager. Follows up on status. Responds to call related to transcription. Checks and returns VM several times throughout shift.

    + Physician training on electronic signature, inbasket and password resets. Submit new physicians to transcription company accurately. Maintain file of who was submitted.

    + Serves as a contact for complaints from physicians regarding quality or timeliness of reports following through as necessary for customer satisfaction. Process all CC copies daily. Distributes to mail room in a neat and organized manner. Addresses envelopes.

    + Orders and checks in charts from Deliverex on-line daily. Distributes to the appropriate staff member or location for processing. (transitional task)

    + Processes surgery schedule daily. Looks up, pulls and orders records from storage, documents location on surgery schedule. Prepares H&P’s for next day surgery. Brings surgery charts and additional H&P to pre-op area by 6:45 am daily. Communicates missing H&P to surgery departments. (transitional tasks)

    + Performs all aspects of document imaging including retrieval, prepping, indexing, scanning and quality assurance. Pulls and files various charts for physicians and various customers. Performs all chart completion functions when needed. Respond to urgent ROI requests via fax. Assists with internal and external customers. Monitors right fax application to insure timely delivery of reports to physician offices.

    EDUCATION

    + _Equivalent experience will be accepted in lieu of the required degree or diploma._

    + HS Diploma or equivalent education/experience

    + Other: Completion of a AHIMA approved transcriptionist program

    + Other: Medical Terminology

    CERTIFICATION & LICENSURE

    + Medical Transcriptionist

    PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

    + 3-5 years recent experience in an inpatient Health information Department.

    + One year work experience merging medical records in electronic environment.

    + One year work experience in electronic health record environment in an inpatient hospital setting.

    + One year work experience as transcriptionist in acute care setting.

    + One year work experience in document imaging.

    SKILLS AND KNOWLEDGE

    + Working knowledge of Medical Terminology and Transcription systems. Transcription support, quality and procedures.

    + Technical skills related to computers, software applications and database maintenance.

    + Proficient in patient identification research in the Epic System. Proficient with reconciling duplicate medical records in an electronic environment

    + Knowledge of Epic In basket. And HIM work queues.

    + Working and demonstrated knowledge of computers in a Window-based environment, Microsoft Outlook a Microsoft Office, Epic (Electronic Health Record) and Hyland Onbase Document Imaging System.

    + Ability to use mouse proficiently.

    + Ability to use keyboard proficiently.

    + Ability to use hot key functions on keyboard

    + Working knowledge of HL7 error messages and Hyperlinks in an electronic environment

    + Able to function in a fast paced environment.

    + Strong analytic skills. Ability to review records and verify patient identification using the EHR.

    + Ability to analyze electronic charts for duplicate medical record numbers.

    + Excellent communication and interpersonal skills.

    + Attention to detail, strong organizational skills.

    + Ability to perform various HIPAA audits.

    + Working knowledge of Medical Terminology and chart order.

    Pay Range is $30.51 to $38.63 / hour

     

    _The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package._ _​_
