    Job Description Summary: Spira Care was designed with our patients at the center of their Care Teams. This collaborative, patient-centered approach to primary care offers patients a one-of-a-kind healthcare experience and offers healthcare professionals an opportunity to really see the impact they can make in their patients’ lives.

     

    Our Care Teams are made up of professionals with diverse backgrounds, experiences, and focus areas who are curious, empathetic, and collaborative. Spira Care is looking to welcome people on our team who are excited about innovating in primary care and are passionate about helping people achieve their health goals.

     

    Triage Nurses must be passionate about infusing “care” into “healthcare” by offering unparalleled support and kindness to our patients. They work onsite to resolve patient concerns with the care team.

    Job Description:

    + Establishes a trusted partnership with the member and builds creditability through knowledge sharing and education around healthcare needs through telephonic assessments, evaluation of symptoms, scheduling as determined by need, and answering member questions while using critical thinking and prioritization in collaboration with the care team.

    + Responsible for incoming STAT calls from the scheduling team. Provides telephonic backup coverage for different care centers based upon the need.

    + Provides a high degree of customer service and medical advice including receiving member incoming calls (return calls and/or triage), in-person interactions, and member outreach as needed in a timely manner for pediatric through geriatric populations.

    + Documents member interactions in the EHR.

    + Completes tasks assigned to inboxes within the EHR system and member call queues.

    + Works with the care team to meet quality performance targets and metrics established for the providers and care center.

    + Serves as a backup RN for the care center to assist in nursing duties as needed.

    + Operates successfully in an ambiguous environment that requires a high degree of adaptability, agility and reliability.

    + Maintains confidentiality of all regulated information in compliance with state and federal laws; reads, understands and follows medical policy and all corporate, divisional and departmental policies and procedures. Including but not limited to: Protected Health Information (PHI) and Personal Identifiable Identification (PII).

    Minimum Qualifications:

    + Associate’s or Bachelor’s degree in nursing.

    + Valid and active Registered Nurse (RN) in Kansas and Missouri or multi-state compact license.

    + BLS certification from American Red Cross or American Heart Association.

    + Maintain licensures and certifications throughout employment.

    + 5 years of healthcare experience.

    + 3 years full-time direct patient care in a clinical/hospital setting.

    + Intermediate knowledge/experience with common diagnoses typically seen in primary care.

    + Ability to work effectively in a fast-paced environment with competing priorities.

    + Intermediate knowledge of Microsoft Windows applications.

    + Ability to work with a variety of member tools/applications used to access and document health information and support member interactions through various communication channels (phone, in-person, chat, email, etc).

    Preferred Qualifications:

    + Bachelor’s degree in nursing.

    + Triage experience in Primary Care.

    + Experience with ClearTriage software.

    + Athena EHR experience.

    + Experience with ACA and/or Medicare populations.

    + Advanced knowledge/experience with common diagnoses typically seen in primary care.
