    Registered Nurse - Outreach CareManager Contract Role

     

    Job Title: RN - OutreachCare Manager

     

    Contract Duration: 6 weeks ( extension options )

     

    Hourly Rate: $35/hr

     

    Start Date: 5/19

     

    Work Schedule: Monday toFriday, 8:00 AM - 5:00 PM

     

    Position Overview: The RN Care Managerwith a focus on outreach will be responsible for engaging with the communitythrough various events and activities. This role involves meeting withcommunity providers, participating in community events at assigned locations,and working at the Day Resource Center (DRC) as needed.

    Key Responsibilities:

    + Conduct basic nursing functions including patient assessment, documentation, and engagement.

    + Manage patient care by monitoring vital signs, overseeing medication management, and providing patient education.

    + Participate in outreach events during the week and occasionally work at the DRC.

    + Collaborate with community providers to enhance patient care and community health.

    Qualifications:

    + Preferred experience RN Care Management

    + Valid RN license in the state of North Carolina.

    + Experience in public health nursing/ clinic preferred.

    + Strong communication and interpersonal skills.

    + Ability to work independently and manage time effectively.
