Description:
GENERAL SUMMARY AND PURPOSE:
Under general supervision, performs a wide variety of functions to support and maintain complete and accurate patient health records for Saint Alphonsus Medical Group. Facilitates the transfer of patient health information between systems, including from paper charts to electronic, or electronic to electronic EMR systems.
Per protocol, identifies the key data elements in the medical record and accurately inputs into the new electronic health record. Supports clinic operations by filing incoming documents into the EMR system to the correct chart location. Performs other tasks as requested to support complete and accurate documentation of patient care.
**SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE** :
High school diploma or equivalent required. Education, training or experience as a Medical Assistant, Unit Clerk, CNA, pharmacy tech, medical record coder, health information technician or similar clinical position preferred.
Knowledge of medical terminology required.
Must have active driving privileges. If, during the course of employment, driving privileges change or colleague receives a driving citation, the colleague is required to notify their supervisor and/or HR.
**ESSENTIAL FUNCTIONS** :
Knows, understands, incorporates, and demonstrates the Organizational Mission, Core Values, and Vision in behaviors, practices, policies and decisions. Abstracts medical information from hard-copy (paper charts) / electronic patient medical records and accurately inputs it into the Electronic Health Record (EHR).
Follows procedures to ensure appropriate review and accuracy of medical information. Prepares, sends, and matches CCD's (Continuity of Care Documents) from NextGen EHR to Cerner EHR, or between other EHRs as necessary. Scans medical record documents and files them correctly into appropriate EHR. Exports / imports medical record documents from multiple electronic health record systems (EHRs) and files them correctly into current EHR. Assists with training new colleagues when applicable. Reviews and audits patient records for quality assurance.
Generates daily appointment schedules or other reports as necessary to perform and prioritize work. Ability to use knowledge and skills with scanning, importing, exporting, data entry in multiple EHR systems to support SAMG work processes as needed. Interacts with physicians, clinic managers and clinic staff when working at clinics as needed.
Uses MS Excel to document and track required daily chart count and protocol. Submits spreadsheet to Supervisor as requested. Communicates effectively and respectfully with Supervisor, Lead, coworkers and clinic staff when applicable.
Protects patients' rights by maintaining confidentiality of personal information and follows guidelines for HIPAA's regulations.
Able to travel to various physician clinics within the SAMG network based upon assignment/schedule. Ability and flexibility to work when and where needed (back-up for multiple clinic locations).
