    35 Hour work week

     

    Hourly wage $20.80 - $22.88 ($37,856 - $41,641.60 a year)

     

    Growth advancement opportunities/career ladder with the first opportunity of a 10% wage increase within 6 months to a year of hire.

    Job Duties:

    + Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program).

    + Support individuals with disabilities to utilize their services and funds to live in the community.

    + Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations.

    + Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence.

    + Document all communications.

    Education:

    + A Bachelor or Master of Social Work degree is preferred.

    + A college degree in Human Services or related field will be considered.

    + A high school diploma or equivalent is required.

    + Experience providing OPWDD funded services or other case management services may be considered in lieu of higher education.

    Knowledge, Skills & Abilities:

    + 2 years of Case Management experience required. Higher education will be considered in lieu of experience.

    + Strong customer service and computer skills.

    + Ability to work in a fast-paced environment.

    + Knowledge of Microsoft Excel, Word, and Outlook.

    + Excellent verbal and written communication skills.

    + Strong organizational skills and attention to detail.

     

    Travel Required: Minimal
