    In accordance with the Mission and Guiding Behaviors; the Physical Therapist provides evaluation and treatment of patients referred for physical therapy services. The Physical Therapist communicates and collaborates with other caregivers to provide quality care and maintains accurate patient records

    Responsibilities

    Exhibits each of the Mount Carmel Service Excellence Behavior Standards holding self and others accountable and role modeling excellence for all to see.     Meets population specific and all other competencies according to department requirements.

     

    Promotes a Culture of Safety by adhering to policy, procedures and plans that are in place to prevent workplace injury, violence or adverse outcome to associates and patients.

     

    Relationship-based Care: Creates a caring and healing environment that keeps the patient and family at the center of care throughout their experience at Mount Carmel following the principles of our interdisciplinary care delivery system.

     

    Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the manager, clinical coordinator or designee.

     

    Documents accurately PT evaluation results, treatment interventions, patient progress toward goals, and discharge records.

     

    Maintains proper billing for the services provided.

     

    Delegates appropriate treatment procedures to the Physical Therapist Assistant/PTA; reviews/co-signs their documentation.

     

    Communicates with appropriate health care providers on matters concerning patient treatment, continuity of care, and discharge planning.

     

    Responsible for effective and efficient use of supplies and equipment. Assists with inventory/re-ordering/recommends new equipment or supplies as assigned.

     

    Coordinates the scheduling of assigned patients with self, peers, and other health professionals.

     

    Supervises and evaluates affiliating students as assigned.

     

    Oversees licensed and unlicensed staff as assigned.

     

    Maintains department readiness (i.e. supplies, linen, and equipment cleaning).

     

    Escalates any unusual situations or problems.

     

    Utilizes effective organizational and time management skills

    Requirements

    + Education: Graduate of an accredited physical therapy program; Doctorate degree preferred.

    + Licensure / Certification: Current license to practice Physical Therapy in the state of Ohio; Current Basic Life Support (BLS) for Health Care Provider certification required.

    + Experience: One year of clinical experience in respective area preferred.

    + ·Effective Communication Skills

    + ·Ability to make decisions and utilize good judgment concerning patient evaluation and treatment.

    + Ability to function in a professional capacity with minimal supervision.

    + Physically able to perform strenuous activities, such as lifting and transferring patients.

    + ·Ability to travel to other sites as necessary to meet system needs.
