JOB SUMMARY

The Social Worker (BSW) – Home Health/Hospice functions as an integral member of an interdisciplinary team that provides supportive services to help meet patient, family, social, emotional and economic needs in relation to the patient’s diagnosis. Social Worker (BSW) – Home Health/Hospice provides psychosocial support for patients, collaboratively working toward program objectives of achieving the best possible quality of life for patient and family. The Social Worker (BSW) - Home Health/Hospice provides services and care in accordance with a comprehensive patient care plan and under the supervision of a Social Worker (MSW) – Home Health/Hospice.

JOB QUALIFICATIONS

EDUCATION

_For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation._

**Minimum Required:** Bachelor’s degree in social work or related field.

**Preferred/Optional:** Master’s degree in social work.

EXPERIENCE

**Minimum Required:** One year of social work experience in a healthcare setting such as clinic, nursing home or hospital.

**Preferred/Optional:** Three years’ experience working as a Social Worker working with a target population, community health setting or in a healthcare facility working with children and adults.

CERTIFICATIONS/LICENSES

_The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position._

Minimum Required:

+ Current Social Worker license to practice without restriction in Wisconsin.

+ Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.

+ Current applicable state driver’s license with acceptable record.

**Preferred/Optional:** Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.

JOB SUMMARY

The Social Worker, Masters of Social Work (MSW) is part of the total healthcare team who works independently as an advanced practice social worker to help meet the complex social, economic and emotional needs of patients and families for specialty and non-specialty populations. The individual employ evidence based practices when conducting bio/psycho/social assessments with patients to aid in identifying the needs, wishes, motivation and strengths of the patient as a participant in their healthcare. The Social Worker, Masters of Social Work (MSW) will coordinate referrals and patient care needs for health services in the home and community. This individual will primarily care for **child/adult/a combination of child and adult** populations when performing the functions of this role.

Flexible daytime hours; 4 to 5 days per week
