About the Role
Rapid Rehousing for Young Adults is a solution to homelessness designed to help young adults aged 18-24 quickly exit homelessness to permanent housing. The program provides rental assistance and support services that are tailored to the unique needs of the young person. The Case Manager will provide case management to a case load of 12-15 youths in the Rapid Rehousing Program.
Responsibilities
What You’ll Do
+ Complete intake process with youth according to established guidelines and make appropriate contact with and/or referrals to other agencies.
+ Complete all need assessments.
+ Develop individualized service plans for each youth and update monthly.
+ Facilitate, develop, and take part in skill building groups.
+ Make referrals for outside services when needed such as substance abuse, mental health, etc.
+ Lead intakes, treatment team meetings and discharge planning meetings.
+ Provide crisis intervention to youth in their apartments and mediate roommate issues as needed.
+ Conduct outreach and presentations as needed and as assigned.
Qualifications
Your Qualifications
+ Bachelor’s degree from an accredited college.
+ 2-5 years of experience of working with adolescents.
+ Knowledge and understanding of the homeless population.
+ Comfortable traveling throughout NYC.

Office / Field
Position Type/Expected Hours of Work: This is a full-time position. Days and hours of work are Monday through Friday in Queens, NY. Work hours options are 9:00 am - 5:00 pm, 9:30 am - 5:30 pm, 10 am - 6 pm.
Additional Requirements:
+ Authorized to work in the U.S.
+ Ability to work in-person in New York City (NY) for at least 3 days or more per week.
+ Ability to travel to other Rising Ground sites (if required)
