    Per Diem Occupational Medicine Advanced Practice Provider in the Phoenix Metro Area

    Our Occupational Medicine Provider will perform baseline and annual physical exams, evaluate, treat, and follow up on industrial injuries and non-industrial medical conditions, infectious and hazardous material exposures.

    Required Qualifications:

    + NP or PA-C designation (AANP/NCCPA)

    + Current prescribing privileges, including a DEA license for controlled substances required

    + Current AZ license or eligibility

    + ACLS/BLS required

    + A minimum of 2 years' experience in Occupational Medicine is required

    + Certification to perform NRCME (DOT) driver examinations required, or acquired within 6 months of employment

    + The ability to read and interpret EKGs and exercise treadmill stress tests is a plus or obtain training within 6 months of employment

    + Per Diem (employed position not 1099 eligible)
