    Description

    Under the shared direction of the Director of Long Island City Outpatient Services, and Director of Recovery Services and Community Outreach, this program seeks to establish screening, brief intervention, and referral to treatment service site at the Brooklyn Community Recovery Center. This independent practitioner screens and assesses participants utilizing a variety of tools, which may include, but are not limited to the Comprehensive Intake Assessment, Modified Mini Screening, LOCADTR3. This practitioner also participates in the coordination of community resources/partnerships to facilitate referrals, and contributes in community engagement work as necessary and/or appropriate.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

     

    Other duties and responsibilities may be assigned.

     

    + Conducts level of care assessments

    + Case conference assessments with medical/psych department plus campus leader

    + Coordinates and completes the admission process to maintain an accurate daily census

    + Assist in making referrals to patients that are not appropriate for our services

    + Coordinates and completes the admission of patients with approval by their authorized representatives and/or funding agencies.

    + Performs assessments, screening, and crisis intervention with patients as required.

    + Evaluates test results, psych evaluations and other reports/interviews to determine if candidate is appropriate for treatment at Phoenix House.

    + Maintains accurate and timely documentation in accordance with the expectations and standards of Phoenix House policy and procedures and state licensing requirements.

    + Participates in clinical reviews, staff meetings, trainings, and other meetings in accordance with program needs.

    + Establish and maintains community agency communications and contacts.

    + Assists in marketing the program and increasing the referral base.

    + Attends all required staff training sessions in accordance with Phoenix House and state licensing requirements.

    + Adheres to and promotes the Phoenix House mission and values. Acts as a role model to patients and other staff.

    + Adheres to the professional ethics and conduct of standards as established by professional licensure and the policies of Phoenix House.

    Qualifications

    REQUIRED EDUCATION

    • Master’s Degree in Social Work, Mental Health Counseling, or related field

    LICENSE/CERTIFICATIONS

    • NYS LCSW, LMHC, LMFT, LMSW in good standing required

    • Diagnostic Privilege preferred

    • CASAC preferred

    REQUIRED EXPERIENCE

    • Minimum three years working in substance use treatment admissions

    • Familiarity with regulations governing mental health and substance use treatment services

    WORK ENVIRONMENT

    • This job primarily operates in an indoor office environment. Some community outreach work and partner coordination may require travel out of the office. This role is an extension of our outpatient program in Long Island City (Queens County) and will require travel to that location. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
