JOB SUMMARY AND PURPOSE
The Care Manager (Specialist) provides intensive care to program participants who have been, or are currently, involved with criminal justice system as well as suffering from co-occurring disorders, meeting participants at least weekly. The Care Manager (Specialist) is the primary point of contact to coordinate services and care for assigned clients, handling clients with the most complex cases of high acuity problems and co-morbidities. This position provides clients with the highest levels of acuity and co-morbidities individualized care through access to service linkages, counseling, treatment planning, and various crisis intervention methods in order to prepare individuals for further success
DUTIES AND RESPONSIBILITIES
+ Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop a detailed plan to meet the high acuity needs, sets goals for the clients, and locates the resources needed to meet the goals.
+ Provide service linkages and support systems to ensure identified needs are met.
+ Coordinates individualized planning and follow-up with clients to meet short and long-term needs, tracks progress on meeting agreed upon goals.
+ Conducts regular meetings with clients; consults with service team regarding progress of cases; records all services provided and case notes, maintaining confidentiality of client-s data in accordance with HIPAA and other relevant funder, local or state regulations.
+ Facilitates support groups, and group education events.
+ Provides crisis management, applying experience and through consistent communication with clients, provides intervention as needed
+ Appears in court or other government agencies as advocate for clients relating to re-entry into society
+ Mentors junior program staff members, participates in professional development events
+ Other duties as may be assigned by Program Manager
ESSENTIAL DUTIES:
+ Assessment: through individual meetings on a regular basis develop a detailed plan of action to meet client needs, set goals and find the necessary resources to meet those goals
+ Ongoing meetings with clients, tracking progress, continuously evaluating client needs
+ Coordinate Care with other service providers, connecting participants to appropriate services
+ Advocacy for clients in court and before other agencies/community settings
+ Facilitates Group support sessions and group education sessions for program participants
+ Mentors program staff, participates in professional development events
+ Maintains hard copy records and electronic files of all services provided to clients, following HIPAA and other related regulations
+ Other duties as required and may be subject to change
Qualifications
REQUIREMENTS:
+ Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
+ Must be able to pass Los Angeles Sherriff Department (LASD) background check
+ Valid California driver's license and have reliable transportation
+ Approval from Department of Health Services (DHS)
+ TB Test
EDUCATION:
+ Bachelor-s Degree in Social Work or related field or a combination of education and relevant experience (four years- total).
EXPERIENCE:
+ Minimum 1 years of experience as a case manager
PREFERRED:
+ Counseling Certification
+ 2 years of experience working with target population
+ Familiar with HMIS, electronic record-keeping
