Job Description:
In this role you will:Be part of an interdisciplinary palliative care team. You will provide psychosocial assessments and support, advanced care planning and advocacy to patients and families in the healthcare setting. This position involves collaboration with community partners such as hospice and outpatient palliative care programs. In addition, you will provide discharge planning services to patients followed by the palliative care team.
Education Qualifications:
A bachelor degree in social work from an accredited social work program.
