• Location: Syracuse, NY

• Job Type: Full-Time

• The anticipated wage for this position is between $70,000 and $75,000 a year. Yearly wage may depend upon experience, education, geographic location and other factors.

• **What You'll Do:**

• Lead, develop, and oversee Social Services Department goals and policies

• Conduct resident assessments and create individualized care plans

• Guide and support residents through care transitions and adjustment periods

• Supervise and mentor social work staff

• Champion discharge planning from day one of admission

• Engage in occasional community outreach

• Utilize your experience, creativity, and judgment to make a lasting impact

• **What We’re Looking For:**

• LSW or MSW required

• 2+ years of leadership experience in long-term care (LTC) preferred

• Strong knowledge of State & Federal regulations

• Proven ability to lead with empathy, professionalism, and strategic thinking

• Excellent communication and interpersonal skills

**Pay Details:** $70,000.00 to $75,000.00 per year
