    Position Overview:

    A core member of the collaborative care team, including the patient’s medical provider, psychiatric consultant, and the larger primary care team or medical team. Responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and other mental health providers.

    **Job Description** :

    EDUCATION:

    + Master's: Degree in a Mental Health discipline

    + OR Graduate of an accredited school of nursing

    + OR Doctorate in Clinical Psychology

    CERTIFICATION & LICENSURE:

    + None if possesses a Master’s degree in a Mental Health Discipline

    + Or RN

    + Or PSY-Psychologist

    TYPICAL EXPERIENCE:

    5 years recent relevant experience

    SKILLS AND KNOWLEDGE:

    + Ability to assess the needs of the patient, provide high quality integrative psychotherapy, guided imagery and other evidenced based approaches as indicated.

    + Knowledge of holistic assessment, care plan development and outcomes tracking methods.

    + Candidate must have desire and ability to work as an active member of an interdisciplinary practice team led by physicians.

    + Required to have knowledge of the most recent Board of Behavioral Science requirements and adhere to them.

    Pay Range is $55.86 to $73.73 / hour
